Acadeum Course Request Form

This form is required for any student to gain approval to take a course through Acadeum and the Council of Independent Colleges consortium. Enrollment in a consortium course requires the permission of the student's advisor, the chair and the dean who oversee the course, and the Vice President for Academics. All requests must follow this process. It cannot be student-initiated. It must be initiated by student's advisor in consultation with the chair of the course being offered. Please be advised that additional tuition may be required. Please provide all of the requested information.

Requests for Acadeum classes (regardless of the add/drop date for the the Teaching Institution) must adhere to CBU's add/drop period and tuition/ fee deadlines when adding Acadeum courses. Requests made after CBU's add/drop deadline are subject to late add/drop fees and pollcy.

Additionally, please inform your students that if they are enrolled in a course via Acadeum, these courses are not included in the Follett All-Access program, so they will be responsible for purchasing and acquiring their own required course material for all Acadeum courses. If they are opted-into the Follett All-Access program, the CBU Business Office will exempt them from the Follett fee AFTER the add/drop deadline.
