STUDENT FLU VACCINATION VERIFICATION FORM

Getting a flu vaccine is more important than ever to protect yourself and the people around you from flu and to help reduce the strain on healthcare systems responding to the COVID-19 pandemic. In fact, the Centers for Disease Control recommends that everyone — with very rare exceptions — should get a seasonal flu vaccine. (For info on the flu and its prevention, visit cdc.gov/flu.)

For that reason and for the continued health and well-being of our community, Christian Brothers University is requiring all students who are taking classes on campus and/or using campus facilities such as the library, computer labs, or athletic facilities during the Fall 2021 semester to provide verification to the University that they have had the flu vaccine administered.

Exemptions
• Students who are completely remote/online are exempt from this requirement but are still required to fill out the online form indicating this status.
• Students seeking an exemption from this vaccination due to a medical reason, or because of a sincerely-held religious belief, will need to upload documentation. For medical exemptions, a physician or advanced practice clinician must attest that the individual’s medical status contraindicates the individual’s vaccination. For religious waivers, individuals will provide information to identify the sincerely held religious belief, practice, or observance that is the basis for the request for waiver, with the signature of and contact information for a religious leader on their professional letterhead. Each request for an exemption will be evaluated individually on a case-by-case basis. Accommodations will be granted when they do not cause undue hardship to the University or pose a direct threat to the health and safety of others.

Deadline
The deadline for submitting this verification is October 15, 2021. Students who do not provide proof of vaccination by this deadline will not be allowed to register for future terms until proper verification is submitted.
