Student Grievance Submission Form

The University has a grievance policy and procedure for any student who may have a complaint with any aspect of the University. Details are to be found in the online student handbook, The Compass, under Section 8, Student Code of Conduct. Any complaint, whether formal or informal, should be filed with the Vice President of Student Affairs - or their designee - who will serve as the primary investigator for cases involving students.

You should always make the effort to talk directly to the person/department identified as the cause of your complaint PRIOR to submitting this form.

This form is used by the student to submit a formal complaint that has not been resolved satisfactorily with the faculty, staff, student, and/or others involved.

Please complete all fields so your complaint may be directed to the proper University officials. All complaints must be submitted electronically by using this form. Submitted forms are processed by the Office for the Vice President for Student Affairs.
